What you\’ll do

Create person-centred support plans and lead a small team to deliver them

Support customers with mental health, substance use, homelessness and isolation

Help people build daily living skills and confidence in their community

Carry out risk assessments, reviews and safeguarding checks

Set up and end tenancies, making sure customers feel safe and supported
