As part of our Central Resource Team, you’ll provide cover across services to ensure customers always receive the best support. Every day, you’ll make a real difference in helping them achieve their goals.
What you’ll do:
- Create and develop support plans with customers and coordinate the support worker team.
- Support individuals with complex needs and vulnerabilities, including homelessness, substance misuse, complex mental health, and social isolation.
- Assist with daily living activities: cooking, shopping, budgeting, tenancy upkeep, and moving to independence.
- Carry out risk assessments, set goals, and complete regular reviews.
- Assist with ending and setting up tenancies for our accommodations.
- Provide person-centred, compassionate support without rushing.

You have:
- Passion for helping customers live their best lives, with a collaborative and detail-focused approach.
- Experience creating person-centred support plans and coordinating referrals.
- Ability to work independently, stay calm under pressure, and show resilience.
- A vehicle insured for business use (mileage paid).
- 37.5 hours per week on a rota, including evenings, weekends, and occasional waking nights.
- Travel across County Durham (Brandon, Consett, West Cornforth, Durham) and one service in Sunderland.
- Confident using technology for support plans, learning, and collaboration.
- Enhanced DBS required (we’ll cover the cost).
