Support Coordinator (Hospital Discharge) Salary £27,500 / Earn £14.06 per hour plus 34 days leave (rising to 39) and health cash plan worth over £1,140. Permanent, Full time (37.5 hpw), Monday to Friday 9am to 5pm at Musgrove Park Hospital Taunton and The Link, Yeovil.
We are brilliant at what we do, and we look after each other just as much as we support our customers. Visiting wards at Musgrove Park Hospital, one day per fortnight at the community hub, The Link in Yeovil.
- Partner with local hospitals and housing teams to reduce the average length of stay on in-patient wards for customers who are homeless or rough sleeping.
- Create person-centred discharge plans with and for your customers ensuring that these are reviewable and sustainable in the long term.
- Collaborate with professionals in health, housing, and other appropriate services to facilitate a smooth transition from hospital to accommodation for customers with complex needs.

Working days are Monday to Friday 9am to 5pm. Able to use technology for creating and updating support plans, complete online learning, and collaborate with colleagues.
You'll need an Enhanced DBS check done and we'll pay for it! A vehicle insured for business use and a valid driving license (we'll pay for business mileage).
