What you'll do
- Carry out person-centred assessments to understand how a customer's health, abilities and environment affect their daily life and independence.
- Work with customers to set meaningful goals and create practical plans that support progress, confidence and wellbeing.
- Help customers develop skills, overcome barriers and take part in everyday activities at home and in their community.
- Provide clear occupational therapy advice and guidance to colleagues, helping them deliver consistent and effective support.
- Work closely with families, carers and partner professionals to make sure support is joined up and customer focused.
- Manage a varied caseload, review progress and maintain clear records that support safe and effective practice.

You have
- A degree in Occupational Therapy, HCPC registration and a strong understanding of occupational therapy and person-centred practice.
- Experience of assessing people with physical health, mental health, cognitive, learning or communication needs.
- Great listening skills and the ability to build positive relationships that put customers at the heart of decisions.
- Confidence working collaboratively with colleagues and partner organisations to achieve the best outcomes for customers.
- Knowledge of professional standards, clinical guidance and balanced risk management approaches.
- A positive, solution-focused approach with a genuine passion for helping people live the life they choose.
The practical bits
Temporary (12 months), full time (37.5 hpw) providing support to vulnerable young people. The office base for this role will be Mercury House; however, the position will involve working across three supported accommodation services located in Brandon, Consett, and Shotton Colliery providing support to vulnerable young people Monday to Friday, 9:00am to 5:00pm. You'll spend one day a week at each supported accommodation service, with the remaining two days supporting Central Resource Teams (CRT) across the region, helping to improve service delivery and share best practice. You need a vehicle insured for business use, and we pay your mileage. You also need an Enhanced with barring list DBS check (we pay).
