Lead Support Worker - Social Care

Overview

Lead Support Worker (known by us as Support Coordinator). Permanent, Full time (37.5 hpw). West Yorkshire – Leeds, Huddersfield, Wakefield area.

Pay £13.21 per hour (£25,838 per annum) plus health cash plan and instant pay access with Stream.

What you’ll do

• Create person-centred support plans and lead a small team to deliver them
• Support customers with mental health, substance use, homelessness and isolation
• Help people build daily living skills and confidence in their community
• Carry out risk assessments, reviews and safeguarding checks
• Set up and end tenancies, making sure customers feel safe and supported

Practical bits

• You’ll work Monday to Sunday on a rota-based system
• You’ll work 37.5 hours per week on a rota-based system
• You’ll cover a patch across services in West Yorkshire
• You need a vehicle insured for business use, and we pay your mileage
• You need an Enhanced DBS check (with barring list) that we pay for
