Join Our Caring Team at Homecarers Care Services. Are you an organised and compassionate professional looking to make a real difference in people's lives? Due to continued growth across Manchester and Tameside, Homecarers Care Services is looking for an experienced Care Coordinator to join our dedicated team. As a Care Coordinator, you will play a critical role in ensuring that service users receive high-quality, person-centred care.
Working closely with the Registered Manager and Trusted Assessor, you will coordinate and manage care schedules for service users considering their individual needs, preferences, and goals. You will be responsible for organising rotas, shift patterns, liaising with health professionals, supporting home visits, and assisting with the creation of care plans.
Daily duties will include:
- Coordinating care schedules and staff rotas
- Matching carers to our service users based on needs and preferences
- Maintaining accurate records and compliance requirements
- Liaising with service users, families, care staff, and healthcare professionals
- Responding to changes in care requirements and operational needs
- Ensuring high standards of care and customer service are maintained
What We’re Looking For:
- Previous experience in care coordination, scheduling, or care management
- Excellent communication and organisational skills
- Ability to work effectively under pressure and manage competing priorities
- Good IT skills and experience using rota or care management systems
- A positive, flexible, and team-focused approach
- Knowledge of the care sector and CQC requirements
- A valid UK driving licence and access to a vehicle is essential
As part of the role, you will participate in a supportive and well-managed alternate weekend on-call rota, providing the opportunity to earn additional income while contributing to the continuity of high-quality care.
Areas Covered: This role supports our growing services across North and South Manchester and Tameside.
