A fantastic opportunity has arisen to join one of the largest expert nursing teams in the UK to deliver first class specialist stoma care in the community. Whether you are a Stoma Care Specialist or a Nurse / District Nurse with a desire to learn a specialised skill, we are looking for someone who will be passionate in supporting a recognised care pathway of stoma patients from the acute into the community.
Location: Covering Ealing, Brent & Harrow
Contract: Permanent, working Monday to Friday, 37.5 hours
Responsibilities:
- The Nurse Specialist will develop and deliver a community-based stoma care service in patient’s own homes as part of a secured partnership completing the following:
- Support patients discharged with a stoma from the assigned hospitals in support of the acute based teams
- Provide safe, effective and specialist nursing care, information and advice to patients and their relatives post stoma surgery and throughout their stoma journey
- Provide teaching and support to patients on how to independently care for their stoma
- Maintain good communication links and co-operation between the acute teams, GPs, Community Nurses and other disciplines / departments
- Ensure timely and accurate patient records are maintained
- Support the introduction of new products and provide opportunities for patients to review
- Participate in discussion for enhancements to the service provided in your territory
- Work with Company Nurse Manager to set annual clinical objectives and track regularly to ensure timely completion
Person Specification:
- Qualified Nurse RGN (essential)
- Community / district nurse experience (desirable)
- Recognised Stoma Care Qualification (desirable)
- Driving Licence essential (home visits will be required)
- Proven experience with an interest within the stoma care / colorectal area
- Evidence of recent post registration training and development

- The anticipated base salary range for this position is £42,000 - £52,000, depending on experience or other legitimate business factors
