About the role
Our Care Co‑ordinators support the practice in delivering excellent patient care by efficiently co‑ordinating patient care, using care navigation to ensure that patients receive the right care at the right time in the right place. In this role you will join our Patient Services Team, where we are committed to delivering high quality patient care, focusing on meeting the individual needs of each and every patient. Your work will make patient interactions with the practice more streamlined and efficient. You will be passionate about making a positive impact on patients’ health and wellbeing, excel in a busy and demanding environment, and will be able to learn our IT systems quickly.
About the practice
We are a large rural practice with over 35,000 patients and operate from seven locations across Holderness. As a single‑practice Primary Care Network, we have a wonderful opportunity to transform care for our patients. Our team includes 23 GPs, an extensive multidisciplinary team of healthcare professionals, and a great patient services team.
