We have an exciting opportunity for a Senior Long Term Conditions Nurse to join our Primary Care team at HMP Winchester, a category B Prison located in central Winchester.

Hours: 37.5 - Monday to Friday 8.30–4.30pm. No weekends or nights.

Responsibilities:

  • Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
  • Identify immediate health needs including any external services or prescribing requirements that prisoners may have received prior to remand or repatriation onto a wing.
  • Facilitate arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient.
  • Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections.
  • Deliver continuity of care across health and social care agencies from courts, to custody and community.

Qualifications:

  • Registered Nurse – General with relevant NMC Registration.
  • Non‑Medical Prescriber / Practitioner / Long‑Term Conditions.
  • Some line management responsibility after probation period.
  • Educated to degree level.
  • Knowledge of health and social care models across the criminal justice system.
  • Knowledge of evidence‑based practice and clinical governance.
  • Experience of assessing the needs of and working with individuals with complex health and social care needs.
  • Experience in a hospital, community or prison environment is desirable.

Salary and Support:

Full‑time, annual salary up to £54,397 per annum FTE, depending on experience.
