Summary
The role of support worker is across our Unscheduled and Planned Care Teams- providing stabilisation interventions to service users in acute distress and ongoing interventions as part of a care plan with service users who are managed under Care Programme Approach. Liaise as appropriate with other members of the multidisciplinary team and other agencies involved in the care of individual service users. The role will also require undertaking and delivering defined activities and interventions for a caseload of identified service users, in accordance with the agreed personal recovery plan. This may be either on a one-to-one basis, or as part of a group activity. See Job Description for full details.
