We are looking for a Clinical Nurse Specialist - Community Palliative Care to join our Team.

What would be my responsibilities as a Clinical Nurse Specialist?

  • The Community Palliative Care Clinical Nurse Specialist (CNS) will work with the Hospice multidisciplinary team to facilitate a supportive framework aimed at enabling patients to be cared for in the home environment of their choice.

They will provide information, specialist advice in symptom control and psychosocial support to patients and families in the community.

  • The post holder will also act as an expert resource and facilitator to educate and train other staff including members of the local primary health care teams and in so doing helping to provide optimum nursing care for patients in their home.

Am I the right person for this role?

  • You will be registered with the NMC
  • Educated to degree level
  • Have experience in palliative and end of life care in a community or inpatient setting
  • Be an excellent communicator
  • Be experienced in working effectively in a MDT
  • You will be a champion of holistic care and always considering what matters to the patient
