Requirements:

  • Bachelor's degree in social work, nursing, or a related field
  • Minimum of 2 years of experience in care management or a related field
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work independently and as part of a team
  • Compassionate and empathetic nature
  • Knowledge of healthcare systems and resources
  • Familiarity with medical terminology and conditions
  • Proficient in Microsoft Office and electronic medical record systems
  • Must pass a background check and drug test
