Responsibilities

  • Provide expertise in clinical medicines management as part of a multi‑disciplinary team.
  • Adhere to the local prescribing formulary and undertake cost‑effective prescribing, including medication optimisation.
  • Undertake holistic structured medication reviews to reduce inappropriate polypharmacy.
  • Manage long‑term conditions, lipid reviews, and DOAC reviews.
  • Review medication orders, provide drug information and monitor patient outcomes to ensure the highest standards of pharmaceutical care.
  • Educate patients on medication administration and potential side effects.
  • Maintain accurate records of prescriptions and clinical interactions.
  • Primarily work from home with occasional in‑person clinics; additional hours may be available on Sundays or evenings.

Qualifications

  • GPhC registration (required)
  • CPPE qualification
  • Independent Prescribing Qualification
