Responsibilities
- Provide expertise in clinical medicines management as part of a multi‑disciplinary team.
- Adhere to the local prescribing formulary and undertake cost‑effective prescribing, including medication optimisation.
- Undertake holistic structured medication reviews to reduce inappropriate polypharmacy.
- Manage long‑term conditions, lipid reviews, and DOAC reviews.
- Review medication orders, provide drug information and monitor patient outcomes to ensure the highest standards of pharmaceutical care.
- Educate patients on medication administration and potential side effects.
- Maintain accurate records of prescriptions and clinical interactions.
- Primarily work from home with occasional in‑person clinics; additional hours may be available on Sundays or evenings.
