As a Housing First Co-ordinator, you will lead the delivery of our Housing First programme, ensuring individuals with complex needs are supported to maintain tenancies and empower their quality of life.
This is a key role where you will coordinate multi-agency support, embed Housing First principles in everyday practice, and work collaboratively as part of a small, dedicated team while also being confident in lone working when required. You will make sure people get the right support, in the right place, at the right time.

Key Responsibilities
- Coordinate support for individuals with complex needs transitioning into and maintaining housing.
- Work collaboratively with partners including health, social care, substance use services, and housing providers.
- Uphold Housing First principles: choice, harm reduction, and person-centred support.
- Monitor and evaluate outcomes to ensure effective interventions.
- Provide guidance and supervision to support staff or peer mentors.
- Maintain accurate records and reporting in line with organisational and funder requirements.
