As a Support Worker in one of our supported accommodation homes, you will play a vital role in providing holistic care, guidance, and support to young individuals transitioning to independent living. Your compassionate approach, understanding of their unique needs, and commitment to fostering a safe and nurturing environment will contribute to their personal growth and development. You will be working in a newly regulated environment to ensure that standards are always upheld.

Key Responsibilities

  • Develop and maintain personalised support plans aligned with LA Care standards. Keep them updated to reflect current needs and goals.
  • Deliver key work sessions in line with support plan goals through one-to-one education and reflective conversations.
  • Provide tools, guidance, and referrals to external agencies to help young people achieve their goals.
  • Work with young people to identify education and career aspirations, connecting them with appropriate resources.
  • Maintain accurate records of progress, behaviours, and interactions, adhering to confidentiality and reporting requirements within the outlined timeframe.
  • Complete daily checklists, cleaning tasks, and review communication logs.
  • Safely manage the running of the home, ensuring all allocated tasks are completed.
  • Maintain cleanliness, presentation, and safety standards within the home.
