As a Support Worker in one of our supported accommodation homes, you will play a vital role in providing holistic care, guidance, and support to young individuals transitioning to independent living. Key Responsibilities
- Develop and maintain personalised support plans aligned with LA Care standards. Keep them updated to reflect current needs and goals.
- Deliver key work sessions in line with support plan goals through one-to-one education and reflective conversations.
- Provide tools, guidance, and referrals to external agencies to help young people achieve their goals.
- Work with young people to identify education and career aspirations, connecting them with appropriate resources.
- Maintain accurate records of progress, behaviours, and interactions, adhering to confidentiality and reporting requirements within the outlined timeframe.
