We are looking for a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. As a key member of the surgery, you will play a vital role in delivering high-quality, patient-centred nursing care. The role of the Practice Nurse is central to modern general practice supporting patients in managing long-term conditions, providing health promotion and preventative care, and delivering essential treatment room services. You will work closely with GPs, other nurses, and the wider multidisciplinary team to improve health outcomes, reduce health inequalities, and ensure patients receive safe, effective, and personalised care. Main duties of the job The core responsibilities of the Practice Nurse are as follows. On occasion, there may be a requirement to carry out tasks not listed below, that are within the reasonable scope of the role. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practice Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health Provide routine nursing care to patients in accordance with clinical based evidence, NICE and NSF To diagnose and manage acute and chronic conditions as required To review patient medications to enhance compliance Co-ordinate the provision of travel medicine services Maintain accurate clinical records in conjunction with existing legislation Ensure read codes are used effectively by all member of the nursing team Ensure appropriate use of the chronic disease registers Prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs To implement vaccination programmes for adults and children Job responsibilities Clinical Duties: Provide nursing care and advice to patients across the lifespan in line with clinical guidelines and protocols. Perform routine procedures including: Cervical screening (smear tests) Childhood and adult immunisations/vaccinations Wound care and dressings Chronic disease management (e.g. asthma, COPD, diabetes, hypertension) NHS health checks and screening programmes Travel health advice and vaccinations Assist in minor surgery Health Promotion
