As a Care Coordinator, you will play a vital role in planning, managing, and delivering exceptional care services. Your key responsibilities will include: Planning, developing, and maintaining person-centred and outcome-based services, ensuring rotas are designed for effective service delivery. Monitoring recruitment needs within your geographical area and communicating weekly updates to the Registered Manager. Managing services in accordance with relevant legislation, ensuring compliance with internal and external quality standards. Developing strong relationships with service users, their families/representatives, and other professionals or agencies involved. Producing accurate electronic and written records, including daily reports, aligned with company policies and GDPR requirements. Participating in the on-call rota to meet operational demands and personally covering care calls in the event of staff shortages. Responding to, and reporting, any concerns or complaints in line with company procedures. Preparing management reports and providing relevant information as required. Liaising with community organisations and stakeholders to strengthen service delivery. About You Youll ideally have: Strong organisational skills. A passion for delivering inclusive care, and the ability to build positive relationships. Previous experience providing support within a care setting, administering medication, and using equipment. Basic IT skills, including proficiency in Microsoft Office packages. Excellent written and verbal communication abilities. Knowledge of risk assessment, support planning, and a person-centred approach. A Level 2/3 Diploma in Care or another relevant qualification, such as a Care Certificate (desirable).
