Support Worker to be at the forefront of providing high-quality, enabling care. Individual Outcome and Wellbeing: Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance. Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.Health and Lifestyle: Support individuals inmaintaininghealthy lifestyles and managing their health needs.Communication:Facilitateeffective communication andmaintaindetailed,person-centered records.Medication Management: Oversee medication processes and support individuals in self-administration.Safety and Environment:Maintaina safe, clean-living environment and adhere to health and safety protocols.Closing date: 31 August 2026
