At HFH Healthcare, we meet the growing need for complex care by providing specialist nurse-led services to adults and children in their own homes, in and around London as well as the Southeast of the country.
As an Adult Nurse Case Manager (Tracheostomy Experienced), you would be responsible for providing face to face support to our client and their families whilst creating personalised care plans for each client. You would work closely with a core team of carers training and supporting them with clinical skills and activities.

- Hounslow/Twickenham
- South London /Surrey
- North London
- Herts & Essex

Key Requirements:
- Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN
- Tracheostomy and ventilator experience desirable
- ITU/HDU experience would be desirable
- Care of the spinal patient knowledge desirable
- Enteral feeding experience essential
- Excellent written & verbal communication skills
- Ability to work flexibly and respond quickly to changing priorities whilst meeting deadlines
- Ability to build strong relationships with clients, their families, and carers
- Supportive management style that can bring out the best in others
- Confident, passionate and possess a "can-do" attitude
- Ability to manage your own caseload of community packages which includes working in the community and at Wimbledon head office
