Care Coordinator (No On call or weekends)
We are looking for an experienced Care Coordinator to join our growing organisation. If you love talking to people and have a knack for problem solving than this could be the role for you! You will play a very important role in ensuring our clients have the right carers in place to provide them with the exceptional care that they deserve.
Key Details:
Salary: £28,000 pa
Location: Wimbledon Head Office Base
Hours: Monday to Friday (full time, in person role)

Role Details:
- This is an exciting time to join our organisation following a period of internal review and improvements.
- You will be responsible for supporting and managing a care portfolio of clients including regular touchpoints and client visits so communication and being confident in a client-facing role is essential.
- You will work closely with your clients and the supporting carers to ensure a stable team of carers is always rostered in and well in advance and ensure that all parties are informed at all times.
- You will be accountable for conducting ongoing carer supervisions and annual appraisals for all carers who work for your clients.
- You will be adept at problem-solving and identifying issues arising to ensure that continuity of care for your clients is always maintained.
- All tasks being delivered will need to be with a mindset of delivering the highest quality care and customer service excellence.
- Due to having an On Call Care Coordinator team, this role does not have any on-call responsibilities and it is not needed to cover shifts in the community.
