HFH Healthcare is a leading provider of nurse-led complex care, delivering specialist clinical services to adults and children in their own homes across London and the Southeast. We are looking for a motivated and compassionate Adult Community Case Manager with experience in tracheostomy and complex care to join our growing clinical team. This is an excellent opportunity for a nurse who enjoys working autonomously in the community while being supported by a highly experienced multidisciplinary team. You will play a key role in ensuring clients with highly complex needs receive safe, effective and person‑centred care that enables them to remain at home.

About the Role

As an Adult Nurse Case Manager, you will manage a caseload of clients with complex clinical needs, providing face‑to‑face clinical support and developing personalised care plans tailored to each individual. You will work closely with clients, families and care teams, delivering clinical training and supporting carers to develop the skills required to deliver high‑quality care safely and confidently. This role combines clinical expertise, relationship management and care coordination, offering the opportunity to make a meaningful impact every day.

Key Responsibilities:

  • Manage a caseload of clients with complex healthcare needs
  • Develop, implement and review personalised care plans
  • Provide face‑to‑face clinical support to clients and families
  • Train and support carers in delivering complex clinical interventions
  • Ensure care delivery meets high standards of safety and quality
  • Work collaboratively with multidisciplinary teams and commissioners
  • Maintain accurate clinical documentation and reports
  • Respond proactively to changes in client needs

Clinical Experience:

We support clients with a range of complex conditions including:

  • Spinal cord injuries
  • Neuromuscular conditions
  • Tracheostomy and ventilation care
  • Acquired brain injuries
  • Learning and physical disabilities
  • Epilepsy

Essential Requirements:

  • Registered Nurse with active NMC PIN
  • Minimum 12 months UK nursing experience
  • Experience in complex care settings
  • Enteral feeding experience essential
  • Tracheostomy and ventilator experience desirable
  • ITU/HDU or spinal care experience desirable
  • Strong clinical assessment and care planning skills
  • Excellent communication and relationship‑building ability
  • Ability to work independently and manage your own caseload
  • Full UK driving licence preferred due to community visits
