Care Coordinator Wimbledon Head Office Base Monday to Friday (full time, in person role)
Salary: £28,000 pa
We are looking for an experienced Care Coordinator to join our growing organisation. If you love talking to people and have a knack for problem solving then this could be the role for you! Role Details (No On Call or Weekends):
This is an exciting time to join our organisation following a period of internal review and improvements. You will be responsible for supporting and managing a care portfolio of clients including regular touchpoints and client visits so communication and being confident in a client-facing role is essential. You will work closely with your clients and the supporting carers to ensure a stable team of carers is always rostered in and well in advance and ensure that all parties are “in the know” at all times.
As part of the ongoing support for your carers you will be accountable for conducting ongoing carer supervisions and annual appraisals for all carers who work for your clients. You will be adept at problem solving and identifying issues arising to ensure that continuity of care for your clients is always maintained. All tasks being delivered will need to be with a mindset of delivering the highest quality care and customer service excellence.
Due to having an On Call Care Coordinator team, this role does not have any on call responsibilities and it is not needed to cover shifts in the community.
