Key responsibilities will include:

  • Undertaking structured medication reviews and supporting medicines optimisation for patients with long-term conditions, multimorbidity and complex medication needs.
  • Identifying and managing medicines-related risks, including polypharmacy, adverse drug reactions and potentially inappropriate prescribing.
  • Supporting the management of frailty, mental health conditions and chronic diseases in collaboration with the wider multidisciplinary team.
  • Providing patient-facing clinical consultations and supporting patients to understand and manage their medicines effectively.
  • Promoting the safe, effective and cost-effective use of medicines in line with current guidance and local prescribing policies.
  • Supporting prescribing safety initiatives, clinical audits, quality improvement projects and pharmacy-led service development.
  • Communicating and collaborating effectively with GPs, nurses, pharmacy technicians and other healthcare professionals to improve patient outcomes.
  • Contributing to the ongoing development of pharmacy services across the PCN.

The successful candidate will be supported to develop their clinical practice, pursue areas of professional interest and contribute to service improvement initiatives in line with their experience and the needs of the organisation.

Person Specification:

  • A sound clinical knowledge base and an interest in medicines optimisation and patient-centred care.
  • Good communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team.
  • Ability to identify medicines-related problems and contribute to safe and effective prescribing.
  • A proactive approach to learning, professional development and service improvement.
  • An interest in developing clinical pharmacy services within primary care.
  • Candidates with experience in general practice or other clinical pharmacy settings are welcome to apply.
  • Support will be available to help the successful candidate develop within the role.

Qualifications:

  • Commitment to continuing professional development and developing clinical skills relevant to general practice.
