Hammersmith and Fulham Partnership PCN is offering a full-time or 2 x Part Time opportunity/ies for motivated and forward-thinking pharmacist/s to take on working within the H&F Partnership EHUB (e-Hub), delivering high-quality clinical pharmacy care to patients who access our GP practice through PATCHES, a leading online consultation platform used across London and the NHS. PATCHES allows patients to submit requests digitally at a time that suits them which creates a steady, clinically rich stream of work that is structured, prioritised and safe.

This role offers exceptional flexibility, with the potential for predominantly remote working. Many pharmacists find this enables better focus, fewer interruptions, and improved work-life balance while still being fully connected to a supportive clinical team. Attendance at practices is required for induction, training and team days, ensuring you remain embedded within the Partnership. You would be welcome to our GP sites whenever wishing or needing.

Main duties of the job

You will manage a wide range of clinical work, including long-term condition reviews, medication optimisation, contraception, HRT, and mental health queries such as anxiety and depression. This will all be from a secure digital workflow that supports good clinical decision-making, continuity and safety-netting. As an Independent Prescriber, you will work with a high degree of autonomy. Triaging, consulting, prescribing and following up patients to resolution. You will be trusted to make clinical decisions, supported by clear pathways and immediate access to GPs and senior pharmacists when needed. You will join a well-established multidisciplinary digital team of GPs, pharmacists and administrators who collectively manage patient requests efficiently and safely.

HFP Medical is a forward thinking PCN incorporating 5 large surgeries in and around Hammersmith. We already have 18 pharmacists and 3 pharmacy technicians amongst our 150 staff who care for 70,000+ patients. Our strong pharmacy team means we can maximise the potential of our pharmacists away from administrative work. We support development of staff and the pharmacy group organise and attend courses of interest to them with guidance from our Head of Pharmacy. HFP Medical PCN is a single GP partnership with 23 partners. Our leadership team includes a Managing Partner, Medical Director, Managing director, Head of Nursing, and Head of Pharmacy. Job responsibilities

  • Practice as an independent prescriber working within scope of practice
  • Manage online consultation requests routed to pharmacy through the EHUB
  • Perform telephone consultations within scope of practice
  • Undertake structured and problem focused medication reviews
  • Manage medication related requests for long term conditions within scope of practice
  • Support medicines optimisation and cost effective prescribing
  • Support contractual working relating to chronic conditions and medication management
  • Identify and elevate complex or high-risk cases to GPs or other clinicians when required
  • Provide advice and support for patients with self-limiting conditions
  • Reconcile medicines following patients discharge from hospital or clinic appointments
  • Manage shared care medication requests
  • Engage with drug monitoring, blood test requests and review
  • Synchronise repeat prescriptions where necessary
  • Link prescriptions to medical conditions
  • Review medications for newly registered patients
  • Provide subject matter expertise on medication for broader clinical team
  • Reauthorise medication for repeat prescribing within scope of practice
  • Contribute to the organisation and running of public health campaigns
  • Communicate effectively and regularly with broader team

EHUB and Systems Working

  • Work across 5 GP practice sites via the shared EHUB Model
  • Use PATCHES online consultation platform, managing queries within 48 hours of receipt
  • Use Systm1 and associated digital systems (AccuRX / Valida) to record accurate clinical documentation, safety net and follow up plans
  • Triageto other clinicians on site for face to face review where necessary
  • Work collaboratively with administrators, GPs, nurses and pharmacy colleagues to ensure safe, efficient and timely care
  • Make suggestions and contribute to quality improvement in EHUB operations and service development

Training, Supervision and Management

  • Complete induction and system training across 5 GP Sites
  • Build strong working relationships with GP practice teams and colleagues
  • Undertake onward training to build skills to further ability to support service delivery
  • Support audit led by EHUB pharmacy lead when required
  • Participate in EHUB related governance projects and research
  • Engage in clinical supervision, appraisal and professional development led by EHUB pharmacy lead
  • Attend EHUB team meetings with GPs, nursing, pharmacy

In addition to the primary responsibilities, the clinical pharmacist is requested to:

  • Keep up to date with latest guidance, supporting practices to conform to NICE, CQC and other relevant standards.
  • Act as the practice point of contact for all medicines related matters, establishing positive working relationships within the EHUB system.

Continuous Professional Development

Take responsibility for own personal development and learning, maintaining a personal portfolio of evidence related to practice, keeping up to date with medical and therapeutic evidence and opinion, legislation and policy developments. Proactively keep up to date with and retain, share and grow knowledge base through regular CPD development.

Person Specification

  • GPhC Registered Pharmacist
  • Experience of Managing Pharmacy Services in Primary Care
  • Minimum of two years working as a Clinical Pharmacist
  • Broad knowledge of General Practice
  • Minor Ailments Certification
  • MUR and repeat dispensing certification
