This newly developed role focuses on medicines optimisation, reviewing hospital discharge summaries, reconciliation, and repeat prescribing within a supportive MDT. You will provide expert medicines advice, lead on safe prescribing, run Structured Medication Reviews, and collaborate with GPs, care homes, and secondary care.
