The service you will work in involves providing high-quality support to an allocated group of service users within the community, carrying out duties in line with organisational policies and procedures.

A day in this role looks like:

  • Clearly articulating the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce support plans.
  • Empowering service users to engage effectively in their liaisons with professionals within the community such as mental health teams, social work teams, housing providers, health professionals, JCP, etc.
  • Working flexibly as part of a team to ensure service coverage and that contractual obligations are met.
  • Developing co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations.
  • Actively monitoring the health, well-being, safety and security of the service and its users, ensuring all concerns are followed up promptly and in line with procedures.
  • Ensuring that administration is done in good time, that detailed, concise and accurate records are created contemporaneously, and that confidentiality is appropriately maintained.
  • Assisting the manager in managing health and safety, as well as repairs and maintenance of the premises, including the garden.
  • Collecting rent and service charges and managing arrears proactively by following policy, protocol and procedure to maintain tenancy and develop independent living skills.
  • Ensuring referrals, assessments and service reviews are effective and timely.
  • Empowering service users to develop independent living skills as part of a planned move-on process.
  • Arranging or referring service users for education, training and employment activities.
  • Assisting the manager in planning, negotiating and managing budgets and making best use of property, equipment and information technology.
  • Empowering a caseload of clients to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations.
