Our mission is to empower individuals to rebuild their lives and achieve independence. Right now, we are looking for a Support Worker to play a pivotal role in our Criminal Justice Service in Brent.

About the Role

  • Empower a caseload of service users to manage their health and wellbeing.
  • Support a staffing rota offering support to service users between the hours of 8am-10pm Mon-Sun each week.
  • Actively monitor wellbeing, mental health, housing and living environment to ensure service users journey into independence.
  • Take a solution focused and co-produced approach to supporting service users with their move-on into independent living.

Sounds great, what will I be doing?

  • Empowering service users to build independence and engage confidently with professionals in the community, such as mental health teams, social workers, housing providers, and employment or training services.
  • Supporting continuous assessments of service users' needs, advocating for appropriate support levels, and helping guide them through transitions in their journey toward independence.
  • Co-producing group activities and peer support networks, encouraging service users to participate in community-based initiatives that align with their goals and aspirations.
  • Managing timely referrals, assessments, and service reviews, while also monitoring the health, wellbeing, and safety of users and responding promptly to concerns.
  • Contributing to daily service operations, supporting colleagues, completing administrative tasks efficiently and confidentially, and following organisational policies.
  • Assisting users in managing their health, tenancy, and financial responsibilities, such as paying rent and service charges.
  • Supporting property management, including health and safety, repairs, and voids, while clearly communicating the aims and model of the service to promote active participation and co-production with service users.
