Sounds great, what will I be doing?
This role provides person-centered support to serviceusers, focusing on increasing independence, maintaining accommodation, and improving quality of life. Responsibilities include creating and reviewing SMART support plans, delivering home or community-based support, facilitating access to medical and community services, and minimizing hospital admissions for mental health. The role also involves working with external agencies, developing interventions for hard-to-engage users, and promoting ongoing engagement with support networks.

The ideal candidate will have experience supporting individuals with mental ill health and complex needs, particularly within community or outreach settings. They should be skilled in helping service users achieve their goals using person-centred, recovery-focused planning.
Strong knowledge of mental health conditionsincluding symptoms, treatments, and crisis responsesis essential, as is an understanding of relevant health and safety practices when working in the community.
The role also requires a solid understanding of equality, diversity, and the importance of upholding service users\’ rights, privacy, dignity, and choice. Knowledge of welfare benefits relevant to the client group and a clear understanding of safeguarding principles and procedures are also essential.
