As a Recovery Outreach Worker, you will provide person-centred, recovery-focused support to adults with severe and enduring mental health needs. The role involves:

  • Managing a caseload of service users
  • Developing and reviewing SMART support plans
  • Delivering outreach support in homes and community settings
  • Promoting independence and wellbeing
  • Facilitating access to health and community services
  • Working collaboratively with professionals and support networks to reduce social isolation and prevent mental health crises or unnecessary hospital admissions

You'll need to be able to demonstrate the core skills this role requires as well as match our values and mission. The ideal candidate will have experience supporting people with mental ill health and complex needs within a community or outreach setting. Strong knowledge of person-centred support planning, mental health conditions, safeguarding, risk assessment, and recovery-focused practice is essential. You should possess excellent communication and relationship-building skills, the ability to engage hard-to-reach individuals, a good understanding of equality, diversity and inclusion, and the capability to empower service users to achieve their goals and improve their quality of life.
