As a Social Worker, you will work as a member of the multi-disciplinary team, providing high-quality social care services to adults of all ages experiencing mental ill-health. The core functions of the role include assessment, care planning, and coordination of care from a social perspective for adults accessing the service. You will deliver recovery-focused care and support to service users and their carers, emphasizing the principles of choice, control, independence, and safety. Additionally, you will act as a champion of social care and safeguarding in your locality.
You will be based on Beech ward. Beech Ward is a 15-bed low-secure unit for men with mental illnesses and a history of offending behaviour who are at significant risk of causing harm to others. It offers specialist psychological, psychiatric, and behavioural therapies, as well as complex specialist rehabilitation to help patients prepare for reintegration into society.
Please note that due to this post being part-time, it does not meet the salary threshold for sponsorship. Therefore, to be eligible for this role, the post holder will require unrestricted right to work.
- Be a champion of social care, including safeguarding, with an understanding of relevant legislation and statutory guidance.
- Be an effective member of the multi-disciplinary team, providing high-quality social care services to service-users and carers.
- Provide assessment, care planning, and coordination of care from a social care perspective.
- Deliver recovery-focused care and support to service-users and their carers, based on the principles of choice, control, independence, and safety.
- Ensure compliance with both child and adult safeguarding policies, including follow-up on safeguarding referrals and child visit requests.
- Liaise with families and carers, supporting nursing staff in arranging visits and developing good relationships.
- Ensure the involvement of service-users and their carers to facilitate service improvements based on their experiences.
- Liaise and negotiate with agencies such as the Ministry of Justice, Police, Children’s Services, Carers’ Networks, and other stakeholders.
- Support colleagues in applying the Mental Capacity Act and ensure its appropriate use.
- Maintain accurate and up-to-date records of all work undertaken.
- Actively promote equality and diversity and challenge discrimination in all its forms.
