Hertfordshire Mind Network is recruiting for a Bounce Back Worker based at Watford General Hospital and HMN centres.
The role focuses on promoting joint working with Watford A&E and the Mental Health Liaison Team to create a seamless pathway into community support for patients recovering from mental ill health.
The post holder will provide up to 16 sessions of emotional and practical support, visit patients on the ward, and continue sessions in the community and in their homes after discharge.
