- East and North Herts Transfer of Care Team – Stevenage (37 hours per week)
- Integrated Discharge Service West – Barnet and Chase Farm Hospital (37 hours per week, 5 days a week)
- Herts Valleys Out of County Hospital Team – 37 hours per week
- Post Hospital Review Team – Hemel Hempstead – 37 hours per week

About The Role
The role involves working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers to ensure safe and timely discharges from hospital. Key responsibilities include:
- Assessing the needs of patients and carers
- Arranging appropriate care packages
- Liaising with professionals and agencies
- Supporting person‑centred discharge planning
We offer a 7‑day service and currently operate a 1‑weekend‑in‑8 rota across the Hospital Team.
About You
Essential Qualifications and Skills:
- Social Work degree
- Registration with Social Work England
- Valid driving licence
