About The Team

0-25 Together Service is a specialist service for disabled children, young people and their families. Our focus is to ensure we support people to make positive decisions in their lives, enabling their independence so that they can take part in their local community.

About The Role

Joining us as a Preparing for Adulthood Practitioner, you’ll be working with young people and their families and carers from the age of 14 to 25. No day is the same. You will manage a caseload and work with a range of professionals involved in a young person’s life. You will be expected to conduct assessments of their needs in line with relevant legislation and review packages of care, work collaboratively with other professionals including schools, colleges, health professionals and SEND. Therefore, you’ll need to be able to work under pressure and be extremely well organised, taking a pro‑active approach to your workload.

  • Knowledge of social care

  • Ability to work alone and under pressure
  • Driving licence and access to a vehicle for work purposes

Driving Required

You’ll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations.

English Fluency

The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).

This job role is Level 9. We welcome applications from candidates who currently reside in the UK with established proof of right to work documentation. We are not able to offer sponsorship at this time for this role.
