Starting Salary: £38,220- £41,771 per annum, plus £1,911 - £2,088 market enhancement pay plus 5% operational allowance per year, plus £1000 welcome payment, relocation and lease car* terms apply - pro rata for part time

Teams available:

  • East and North Herts Transfer of Care Team – 37 hours per week (not accepting AYSE Social Workers)
  • Integrated Discharge Service West: Barnet and Chase Farm Hospital – 37 hours per week, 5 days a week
  • Herts Valleys Out of County Hospital Team – 37 hours per week
  • Post Hospital Review Team – Hemel Hempstead – 37 hours per week

About the role

You’ll ensure safe and timely discharges from hospital by:

  • Assessing the needs of patients and carers
  • Arranging appropriate care packages
  • Liaising with professionals and agencies
  • Supporting person-centred discharge planning

We offer a 7‑day service and currently operate a 1‑weekend‑in‑8 rota across the Hospital Team.

About you

Essential qualifications and skills:

  • Social Work degree
  • Registration with Social Work England
  • Driving licence
