Responsibilities:
Carry out assessments to support safe and timely discharges. Collaborate closely with the hospital Discharge Team and care providers to deliver effective person‑centred care in line with Hertfordshire’s Connected Lives principles. Apply sound judgement and a solution‑focused approach to help patients return home quickly with appropriate support, promoting independence and wellbeing. Work 7 days 1 in every 8 weeks. - Transfer of Care Team – Lister Hospital
