The predicted salary is between 42839 - 47181 £ per year.
Starting Salary: £42,839 progressing to £47,181 per annum (pro rata for part time)
Hours: 37 hours average per week over the year working 12 shifts (11.5 hours paid) over 7 days, 365 days a year covering 8am-8pm. The role will include working a share of bank holidays and weekends amounting to 1 in 3 whole weekends.

About The Team
The Early Intervention Vehicle (EIV) is an innovative and highly regarded service providing integrated health and social care urgent assessment and intervention as part of local 2 hour Urgent Community Response service. Co-located with health colleagues at HUC, the team attend to residents of Hertfordshire over the age of 18 at risk of avoidable hospital admission.
About The Role
EIV Practitioners are interdisciplinary professionals with a focus on crisis management, social care assessment, functional assessment and equipment provision within the context of urgent response. Working in close partnership with Urgent Care Practitioners (UCPs) from HUC to form a 2 person team, you will collaborate with your health colleague to devise care plans and work effectively across teams and agencies within health, social care and community services to manage risk, support onward care and prevent further crisis episodes. Underpinned by Hertfordshire's Connected Lives Model, you will ensure adherence with local authority statutory duties, recording standards and key legal frameworks and able to apply these effectively within the context of urgent and crisis situations, advocating for person centred and risk positive solutions for residents and carers.
Providing the primary UCR falls response for the area, the moving and handling of residents including assisting them from the floor using appropriate equipment is a core component of the role. EIV Practitioners also attend to individuals with complex needs who may be acutely unwell, injured, or in distress. The role and nature of the service also involves direct physical support, and you may occasionally be required to assist with personal care tasks in urgent situations. This role is well suited to a dynamic and assertive professional, looking for a unique challenge. You will require sound crisis assessment skills and have a passion for working with people to optimise their wellbeing in their own home. You must be confident working autonomously, acting as lead social care professional in high-risk circumstances and providing information and advice to residents, families, UCPs and other professionals.
With a focus on crisis management, falls response and frailty, you will work closely with health colleagues on the front line utilising skills in risk assessment and analysis in complex circumstances ensuring proactive identification and onward referrals into frailty pathways. As a level 11 practitioner, you will be expected to be a senior member of the team, providing advice and point of escalation for other team members and UCPs. You will also be expected to be a proactive team member and support the manager in the development and continued evaluation of the service and may be expected to supervise another practitioner.
About You
- Essential: Experience, and registered Occupational Therapist, Social Worker or Physiotherapist
- Full driving licence
- Expected to have a specialist function such as practice educator, BIA, AMHP or specialist moving and handling advisor qualification or be willing to work towards this.
- Ability to work autonomously
- Desirable: Experience supervising or mentoring others would be advantageous.
