Overview

Hertfordshire County Council is looking for Social Workers registered with Social Work England to join our adult social care services. Hospital Integrated Discharge Team Responsibilities

You\’ll ensure safe and timely discharges from hospital by:

  • Assessing the needs of patients and carers
  • Arranging appropriate care packages
  • Liaising with professionals and agencies
  • Supporting person‑centred discharge planning

About You – Essential Qualifications

  • Social Work degree
  • Registration with Social Work England
  • Driving licence

This role is within the Health & Social Care, level 10 job profile.
