Job Title: Hospital Social Worker
Starting Salary: £42,327 – £45,716* per annum (pro rata for part time) – pay award pending

About The Team
Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.
We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.
About The Role
You\’ll ensure safe and timely discharges from hospital by:
- Assessing the needs of patients and carers
- Arranging appropriate care packages
- Liaising with professionals and agencies
- Supporting person-centred discharge planning
We have posts available in:
- Barnet & Chase Farm Hospital – within ULEZ zone
- Watford General Hospital
- Herts Valleys Community Hospital Team – 24 hours per week
We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team
About You
Essential:
- Social Work degree
- Registration with Social Work England
- Driving licence
