Lower Lea Valley Integrated Community team have an exciting opportunity for a Band 6 Continuing Health Care (CHC) nurse to join our team. Lower Lea Valley Integrated Community Team (ICT) are seeking motivated and enthusiastic Registered Nurse to join our friendly, supportive and Gold Accredited Team.

You will be working alongside our health care assistants, qualified nurses and therapy staff to complete assessments and reviews within the Continuing Health Care framework including checklists, full assessments and fast track assessments. You will be liaising with multiple professionals to complete your assessments and have a close working relationship with the CHC team, GPs and ICT team.

Our ideal team member will be enthusiastic, motivated with excellent clinical assessment skills as well as verbal and written communication skills. You will be responsible for managing your own caseload and meeting deadlines for assessments and reviews, therefore excellent time management skills are essential.

A full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Our CHC Nurse role entails visiting patients in their own homes to assess for suitability for Continuing Health Care provision, and completing all necessary paperwork. You will be dealing directly with the Continuing Health Care team to ensure necessary decisions are made promptly for patient care.
