A vacancy has arisen for a Care Co-Ordinator to work at the St James Court. The role is to support people to live valued lives within the community, through the provision of a person-centered approach to the person’s personal, emotional and physical care, working on a shift-work basis. You will ensure that the management of care and support is delivered in accordance with the tenants’ person-centered plan so as to promote independence and choice in each tenant’s lifestyle.
Location: Hazlemere Lodge, High Wycombe, Bucks
Hours: 38 hours per week
Salary: £10.51 per hour
Requirements:
- You must have a driving license and have access to a car.
- You will need a QCF Diploma level 2 / NVQ Level 2 in Health & Social Care and evidence of working towards Level 3.
- You must have 2 years’ experience working within a domiciliary care environment in a supervisory position.
- Good communication skills are essential as they will help you to build positive relationships with our tenants, their families and our staff.
- Ideally you have previous experience of working in a supervisory or management role.
- You will need to demonstrate an understanding of the needs of older people living independently in their own home and how to support them to lead valued lives in the community.
