About you

Working with the Care Team, you will be passionate about supporting those living with Dementia and leading the way in good quality care. We want you to shift the focus from task led care to a more feelings based approach and be a role model for the rest of the team. You will see, hear and feel the difference you are making!

The role

This is a brand new role for us here at Holmers House and you will be the heart of the home.

  • Monitor staff interaction and wellbeing of people living in the home
  • Coach the team around Dementia care, best practice and making each interaction a personal one
  • Create the theme of the day (this is crucial to helping triggers memories)
  • Support Team Leaders to develop and implement person centered care plans
  • Co-ordinate with other professionals to achieve positive outcomes for people living in the home
  • Be a champion of choice; encouraging people in the home to make their own decisions
  • Inspire families and visitors to get involved in the running of the home
  • Show prospective residents and families around the home
  • Organise a warm reception for all new residents
  • Work with all people living and working in the home to develop and maintain our social program
  • Supervise and participate in the mealtime experience
  • Respond to staff feedback and find solutions for any potential problem areas
  • Oversee documentation, supporting and guiding the team around record keeping
  • Provide supervisory and support for staff, placing team appropriately and ensuring a high level of care is provided to residents on a permanent basis
  • Attend training sessions in order to extend personal knowledge in relation to dementia care and best practice
