Operate as part of a multidisciplinary team providing Crisis Intervention and Intensive Home Treatment to CYP across Herefordshire. Use your initiative and take responsibility for yourself and the quality of your work and the service you provide to CYP. Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies, families and children and young people. Actively and creatively engage CYP and carers encouraging them to make appropriate use of the services available as part of a process of recovery and crisis management. Undertake full psychosocial assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure. Undertake acute risk assessment and safety planning for extremely vulnerable client groups during times of crisis. Deliver high quality, evidence-based care to a defined team caseload.
