We are looking for a passionate nurse who is keen to help shape a new way of delivering community nursing care. We are a multidisciplinary team of creative health care professionals who are focused on delivering compassionate, responsive and patient centred care to meet the needs of our patients. This is a full time role. Main duties of the job The role will require: The ability to communicate effectively with others at all levels. Flexible working to deliver the service across the local area. The ability to problem‑solve and manage time effectively. Act as a role model to colleagues. Support with the education of students from both nursing and therapy professions. Responsibility for the initial assessment and review of patients, across a broad spectrum of clinical conditions, considering the holistic needs of patients. Planning, implementing and evaluation of a person‑centred care plan. Provide skilled clinical care, treatment, advice, and interventions, including appropriate transcribing/prescribing of medication. Undertake continuing health care fast track applications and checklists of on‑going nursing need and where eligible commission packages of care from external agencies. Be involved in triage, allocation of work and leadership of other staff.
