Job summary
An opportunity has arisen for a Band3 health care assistant to join our busy neighbourhood team.
You need to be a motivated, flexible and resourceful, caringand compassionate. You will be supported by a highly skilled multi-disciplinaryteam in order to deliver high quality care, whilst supporting the patients witha variety of conditions and needs, within their own homes.
We are looking for a candidate with excellent communicationskills as you will work closely with other professionals, outside agencies and patientsfamilies.
It is an essential requirement thatthe post holder will hold a full UK driving licence, have business motorinsurance and have access to a vehicle.

This is a part time post working 30 hours per week.
Please note the Trust will not routinelyprovide sponsorship to work for this role.In addition to meet Home Office salary threshold you must have thesignificant experience to be offered top of the pay band AND work a minimum of35.5 hours per week.
Main duties of the job
Under the direction of qualified staff, you will engage withindividual patients to build a rapport and enable them to achieve maximumindependence with activities of daily living, which may include:
Washing and dressing, grooming, meal preparation, eating anddrinking and functional transfers i.e. on/off toilet, bed, chair.
Carrying out remedial activities to develop physical, perceptualemotional and cognitive function.
Following appropriate training and competency assessment, carryout specific tasks delegated by the qualified nurse or therapist, such as woundcare/dressings/insulin administration and medication administration in linewith the Trust policy.
Fit and demonstrate the safe use of equipment, as directed by thequalified clinician.
Work withother professionals within the multi-disciplinary team.
Liaise withexternal parties, such as social workers and GPs.
