Job Title: Education, Health and Care Plan Co-ordinator
Reports to: Programme Manager Additional Support
Overall Responsibilities:
To ensure the delivery of high quality support for students with Education, Health and Care Plans (EHCP) receiving High Needs Funding (HNF).
- To undertake students annual reviews in line with statutory duties
- To undertake training across curriculum areas to support the needs of students with EHCP and HNF
- To maintain close links with curriculum areas to support and review students progress on an ongoing basis
- To support students transition into college and to support progression opportunities in line with student aspirations
Main Duties:
- To promote opportunities for transitioning students to access the college prior to the start of their programme
- To liaise with curriculum areas on an ongoing basis to ensure students achievement towards their programmes of study and EHCP outcomes
- To undertake termly reviews with the student and curriculum area to support achievement
- To liaise with curriculum staff to raise awareness of the needs of students supported by EHCP s with HNF through ongoing staff training and awareness raising activities
- To ensure that students EHCP outcomes are shared with curriculum areas to support achievement towards them
- To provide regular opportunities for mentoring identified students as required.
- To prepare students for attendance and participation within reviews utilising a Person Centred approach
- To ensure that reports and annual reviews are written and collated in line with statutory duties
- To lead annual reviews to support student success
- To identify the appropriate level of support required and complete reports to a professional standard for submission to Local Authority funding panels
- To undertake in class observations to ensure that support is effectively utilised and information is used within forward planning
- To feed into curriculum area Performance Monitoring and Self-Assessment Reports
- To develop effective professional relationships with students and teaching staff, working within the College s professional boundaries policy
- To take part in marketing and promotion activities including Open Days, parents evenings and taster events
- To participate in and contribute effectively to tutor meetings and course team meetings
- To develop an excellent working knowledge of the processes around students with EHCP and HNF
- To abide by College policies and procedures, in particular in relation to promoting and safeguarding the wellbeing of students
- To undertake training including qualifications appropriate to the tutor role
