We're looking for a Registered Nurse to join Routes Healthcare as a Nurse Case Manager, overseeing complex care packages and leading clinical excellence across our Yorkshire services.

Your role will involve:
- Managing complex care packages clinically and holistically
- Developing and reviewing care plans and risk assessments
- Leading clinical training and competency assessments for your care team
- Attending MDT meetings and engaging with commissioning groups
- Auditing care notes and medication records
What we're looking for:
- Registered Nurse with a valid NMC pin
- Experience in high dependency or critical care
- Tracheostomy and ventilation experience (community setting desirable)
- Strong communication, organisation, and IT skills
- Understanding of CQC regulations
- Car owner/driver
