We're looking for a Registered Nurse to join Routes Healthcare as a Nurse Case Manager, overseeing complex care packages and leading clinical excellence across our Yorkshire services.

Your role will involve:

  • Managing complex care packages clinically and holistically
  • Developing and reviewing care plans and risk assessments
  • Leading clinical training and competency assessments for your care team
  • Attending MDT meetings and engaging with commissioning groups
  • Auditing care notes and medication records

What we're looking for:

  • Registered Nurse with a valid NMC pin
  • Experience in high dependency or critical care
  • Tracheostomy and ventilation experience (community setting desirable)
  • Strong communication, organisation, and IT skills
  • Understanding of CQC regulations
  • Car owner/driver
