We’re looking for a Complex Case Manager keen to make a real difference for our clients and foster great morale within their care team. You’ll be a role model, empowering the team to deliver high‑quality, person‑centred care.
Responsibilities
- Take ownership of a caseload of complex clients with clinical interventions including spinal injuries, acquired brain injuries, motor neuron disease, and multiple sclerosis, managing their care package holistically.
- Co‑ordinate and lead the transition, start‑up, and continuity of all complex care packages; involve the client, MDT, manage clinical training needs of the care team, and maintain appropriate skill mix to ensure safe and effective clinical practice.
- Uphold high standards in care, prioritising a person‑centred approach and individual wellbeing.
- Act as a responsive advocate for complaints and safeguarding, encouraging improvement.
- Develop comprehensive care plans and risk assessments.
- Provide support for care packages through observations and mentoring sessions, and conduct regular reviews.
Qualifications
