Routes Healthcare are looking for a Care Manager to join their Barnsley team.

You'll lead the day-to-day running of our Barnsley domiciliary care service, managing the team and ensuring service users receive safe, person-centred care in their own homes.

We're looking for someone with care management experience, solid supervisory skills, and a head for quality and compliance.

Your role will involve

  • Overseeing care delivery, call times, and continuity of care for your service users
  • Developing and reviewing care plans and risk assessments
  • Managing and supervising a small care worker team, including rosters and spot checks
  • Maintaining CQC compliance, handling incidents, complaints, and safeguarding concerns
  • Supporting internal audits of care records and medication records
  • Liaising with commissioners, social workers, and health professionals
  • Participating in the out-of-hours on-call rota

What we're looking for

  • NVQ/QCF Level 3 Diploma in Health and Social Care (essential)
  • Experience in domiciliary or community care
  • Experience supervising or managing care staff, including rosters and performance management
  • Working knowledge of CQC regulations and safeguarding procedures
  • Confident using care systems and standard office software
  • Full UK driving licence and access to a vehicle (essential)
