We're looking for a compassionate and proactive Housing Support Coach to support vulnerable residents in sustaining their tenancies and living independently. In this role you'll work directly with individuals to improve wellbeing, address challenges such as hoarding and property condition, and reduce risks within the home. Through practical support, coaching and partnership working, you'll empower residents to build confidence, develop life skills, and maintain safe, secure homes.

A full UK driving licence and access to your own vehicle is essential to the role.

Specifically, in the role you will:

  • Engage directly with Hedyn residents, providing a range of interventions that address the issues that may threaten their tenancy or housing status and that help tackle associated health and wellbeing issues.
  • Work with customers to improve their homes and personal/family safety, money maximisation, affordable warmth, health and wellbeing outcomes and basic living needs.
  • Provide low level intensive support to residents who hoard or are struggling to sustain their home and together develop a person-centred support plan to reduce risk of harm.
  • Provide information, advice, and support to help residents reach positive outcomes with their hoarding, property condition and health and wellbeing.
  • Provide practical help to individuals who may want to dispose of some items in their home.
  • Promote healthy lifestyles liaising with the appropriate agencies as necessary.
  • Coordinate and participate in events and meetings to provide support advice and guidance.
  • Work closely with colleagues and external partners to advocate for the customer and achieve shared outcomes in making the home safe and reducing risk.
  • Ensure the completion of support plans, positive risk management plans and case notes for those supported.
  • Work with residents to develop and deliver a range of hoarding, property, wellbeing and support interventions to enable them to sustain their tenancies and live independently within their home.
  • Using a strength based, coaching and mentoring approach that motivates and encourages residents to find the right solutions to hoarding, property condition or wellbeing issues they may be experiencing.
  • Using a casework approach deliver support to customers who may be experiencing issues with home/personal/family safety, money maximisation, affordable warmth, health and wellbeing and basic living needs.
  • Using the best contact method including phone/email/text and face to face visits at home or at agreed surgery venues engage with customers on support and wellbeing matters.
  • Provide and coordinate effective practical support and assistance to residents on required aspects of general housing support, hoarding, property condition, household management, and low-level budgeting.
  • Collaborate with colleagues to identify support needs using a risk assessment approach, using the most appropriate internal and external referral routes to ensure needs are met.
  • Work with Housing Support Advisors to implement a broad range of pro-active solutions to reduce the risks caused by hoarding behaviour and identify areas of preventative work to minimise risk.
  • Have a working knowledge and understanding of welfare benefits to identify cases for referral.
  • Liaise, signpost and advocate on behalf of customers with other agencies such as Social Services, Environmental health and other health and wellbeing services.
  • Support residents to move from their homes in emergency situations such as fire and flood through our emergency decant process.
  • Maintain accurate records using systems such as CRM to record customer and outcome data to track the 'distance travelled' by customers to highlight the success of interventions and identify potential areas of improvement to services.
  • Ensure safeguarding policies and procedures are adhered to and actively promote safeguarding across the organisation.
  • Publicise and participate in promoting the work of sustaining our customers tenancies by attending events, publicising via social media, or creating news articles.
  • Promote opportunities for customers, stakeholders, and partners to engage and feedback to improve and develop services and enhance communities.
  • Adherence to the associations policies and procedures including, safeguarding, health and safety, GDPR and code of conduct.
  • Actively promote equality and diversity and inclusion in all activities.
  • Undertake other duties, commensurate with the role, as required by the Housing Support Manager to further the organisation and individual objectives and policies.
